Rhonda Jan Cook was born on November 23, 1957. She passed away on January 25, 2011. She is survived by her husband Jeffrey Cook, daughter April Cook, mother Jan Brady, sister Patricia (Donald) Sanislo, step-mother Ellie Brady, and brother-in-law Greg Cook.
